W-BEN, again

Hi;
I received an informative response from Dan Reinhold who has a very interesting site. I am a Canadian and I was wondering if any Canadians can advise me how they completed the W-BEN form.

The form is very simple... although it looks confusing, I'll admit.

Part 1 is straight forward - name, address, etc.

Check off the one that applies in Part II. I check a. and indicate resident of Canada.

Then sign it in Part IV.

And that's about it... but I would go over and read the thing in full as your situation might be different than mine.
