Lead Capture Page (A.K.A. Squeeze Page)

Hello:
I am new at affiliate marketing and I want to do things right. I am always hearing about how important a list is. I want to create a lead capture page before I send my prospects off to an affiliate link. But try as I might I am not able to find a clear step by step guide on how to create one. Oh sure I know about Forms and in fact I even have an example of the small form asking for Name and email but I have no idea how to tweak it so that it works for me. Is there anyone who can clearly describe what needs to be done to get this small but all so important code to function? or maybe provide info on a great how to product?
